Male fertility and its examination
At Felicitas Mehiläinen, we offer expert support and comprehensive examinations for questions related to male fertility.
Oxidative stress occurs when more harmful oxygen compounds are formed in semen than the body can counteract with its own antioxidant defences. This imbalance can damage sperm. Although a small amount of oxidative stress is necessary for the normal function of sperm, an excessive amount can damage the surface structures of sperm. It can also impair their motility and damage the genetic material, i.e. DNA. These changes can have an effect on sperm function and fertilisation capacity.
Oxidative stress can be investigated with a MiOXSYS examination. The examination provides additional information about the quality of semen and complements traditional semen analysis. A physician assesses the need for the examination as part of the investigation of infertility.
A MiOXSYS examination can be considered when pregnancy has not started as desired and male fertility is being investigated. A traditional semen analysis provides important information about the quality of semen, but it does not always reveal all factors affecting fertility.
The examination can be particularly beneficial when the semen analysis result is normal, but the cause of infertility remains unclear. A MiOXSYS examination can also be considered after an abnormal semen analysis. The examination can also be useful if there are underlying risk factors associated with increased oxidative stress, such as varicoceles, inflammations, general diseases, or an age over 40.
It is advisable to seek fertility examinations if pregnancy has not started after a year of regular attempts. The physician assesses at the appointment whether a MiOXSYS examination is indicated.
Abstinence from ejaculation for 1–3 days is recommended before sampling. No other special preparation is usually needed. Only a small amount of semen is needed for the MiOXSYS examination, and the examination can often be performed on the same sample as a traditional semen analysis.
However, follow the sampling instructions provided by the physician or clinic to ensure that the examination result is as reliable as possible.
A semen sample is taken either at our clinic or at home in accordance with the detailed instructions provided, ensuring the quality and reliability of the sample.
If the infertility specialist considers the MiOXSYS examination indicated, the level of oxidative stress is measured from the semen sample. The examination provides valuable additional information about the condition of the sperm.
The physician will go through the results with you and explain what they mean for your fertility. At the same time, you can ask questions about the examination results.
Based on the examination results, the physician will assess whether it is necessary to reduce oxidative stress and what kind of measures could be useful. If necessary, an individual treatment plan is prepared, which may include lifestyle recommendations, such as reducing smoking and alcohol consumption, as well as dietary changes.
In some situations, the use of antioxidant supplements may also be considered to reduce the amount of oxidative stress. The physician will explain the different options and, if necessary, refer you to further examinations and treatment.

Oxidative stress can be caused by many internal and external factors. External influencing factors include, for example, air pollution, smoking, heavy alcohol consumption and psychological stress.
Among internal factors, varicocele, infections and certain general diseases, such as diabetes, can increase oxidative stress. A man's aging also increases the amount of oxidative stress in the semen.
The MiOXSYS examination measures the level of oxidative stress in semen, which is an important indicator of sperm well-being. It helps to identify whether there is excessive oxidative stress in the semen, which can impair fertility. The examination provides valuable additional information alongside a traditional semen analysis.
Oxidative stress can be affected by many factors, and its amount can vary over time. An individual examination result is therefore assessed as part of other health status, lifestyle, and fertility-related examinations.
Yes, in many situations, the amount of oxidative stress can be influenced and the situation corrected. The results of the MiOXSYS examination help the physician to better understand the causes of infertility and tailor the treatment.
Possible health- and lifestyle-related factors are reviewed with the physician to reduce oxidative stress. In addition, the use of an antioxidant supplement can be started to aim at reducing the amount of oxidative stress.
Sperm formation takes several weeks, so the effects of lifestyle changes are not immediately visible. Possible changes in semen quality are usually assessed only after several months.
