Infertility can evoke many questions and strong emotions. The situation is individual for everyone, and finding answers alone is not always easy. It is estimated that about one in five couples experiences infertility at some point in their lives.
Infertility examinations are used to investigate why pregnancy does not occur despite the desire for one. The examinations help with understanding one's own fertility and mapping out potential solutions to realize dreams of having a family.
It is advisable to start infertility examinations at the latest when pregnancy has been attempted unsuccessfully for a year. You can also seek examinations earlier if you have concerns about your fertility or are aware of factors affecting it. A physician will help assess when examinations are topical.

Basic infertility examinations investigate what may be causing the difficulties in getting pregnant and what the chances of starting a pregnancy are. The examinations evaluate the fertility of both partners so that potential factors affecting the onset of pregnancy can be identified and appropriate follow-up actions planned.
For women, an ultrasound scan is usually performed vaginally to examine the uterus and ovaries. If necessary, blood tests and hormone determinations are used to investigate, for example, hormonal function and ovarian function. An examination of the patency of the fallopian tubes, in turn, investigates whether the egg can travel to the uterus.
Male fertility is evaluated with a semen analysis, which examines, among other things, the count, motility, and morphology of sperm. If necessary, examinations are supplemented with blood tests or an ultrasound scan of the testicles.
Infertility examinations usually begin with an infertility specialist’s first appointment or a free first appointment with an infertility nurse. A one-hour appointment is reserved for the first appointment. It is recommended to attend together with a partner, if applicable, so that the situation of both partners can be taken into account.
You can prepare for the appointment by compiling information in advance regarding your health status, previous illnesses, medications, and any possible previous pregnancies. If your menstrual cycle is regular, you can also record the length of the cycle and the start date of your last period.
Also consider what you would like to ask during the appointment or what you would like to receive more information about. This makes it easier for the physician to form an overall picture of your situation and plan suitable follow-up examinations.
An ultrasound scan is performed on the woman through the vagina. Depending on the situation, the examinations are supplemented with basic blood tests and hormone tests. Basic blood tests can examine, for example, haemoglobin, vitamin D, ferritin, and thyroid levels.
If necessary, hormone function can be investigated more extensively, in which case examinations may include, for example, AMH, or anti-Müllerian hormone, which provides indications of the egg reserve of the ovaries. The patency of the fallopian tubes can also be examined if necessary.
As a basic examination, a semen analysis is performed to evaluate the count, motility, and structure of sperm. If necessary, sperm DNA fragmentation or oxidative stress can also be investigated.
After the basic examinations, the physician will go through the results with you and answer your questions. The treatment options will be reviewed and an individual treatment path will be planned for you. In most cases, treatment can be started immediately. Sometimes, however, further examinations are needed.
If the initial examinations give cause, more detailed investigations can be carried out, such as chromosome tests, thrombophilia testing and screening for autoimmune diseases. Often, there is only cause for these examinations if infertility treatments have not been successful or miscarriages occur repeatedly.
Once the examination results are ready, you will review them together with a physician and discuss potential follow-up measures. Depending on your situation, suitable treatment options may include, for example, ovulation induction, insemination, or in vitro fertilisation treatment. If necessary, donated gametes can also be used.
In addition to the examination results, the choice of suitable treatment is influenced by your age, your health status, and previous pregnancies or treatments. The physician helps you understand the different options and find a suitable way forward for your situation. You do not need to know the answers beforehand – the most important thing is moving forward together towards the outcome you hope for.
