It is always advisable to seek examinations earlier if there is a known factor reducing fertility. In women, these can include, for example, being over 35 years of age, endometriosis, or an irregular menstrual cycle. The absence of menstruation, a previous uterine or fallopian tube infection, and anatomical abnormalities can also affect fertility.
A man should undergo a semen analysis already when planning a pregnancy if there is a known history of untreated undescended testes, genital surgery, inflammation, or trauma. A woman's or man's previous cancer treatment is also a reason for fertility examinations.
Investigating the causes of infertility always begins with a careful interview, which forms the basis for the examinations. During the appointment, health status, previous illnesses, possible pregnancies and infertility treatments, as well as lifestyles such as smoking and weight management, are reviewed. In addition, family history and other factors affecting fertility are mapped out. In the examinations, the fertility of both partners is assessed, as difficulties in getting pregnant may involve factors related to the woman, the man, or both.
A transvaginal ultrasound scan is performed on the woman, and if necessary, the examinations are supplemented with blood tests, hormone determinations, and a fallopian tube patency test. A semen analysis is always carried out for the man. Depending on the situation, blood tests and a testicular ultrasound scan are also performed.
Fertility examinations usually begin with a first appointment with an infertility specialist or a free first appointment with an infertility nurse. A one-hour appointment is booked for the first appointment. It is recommended to come together with a partner, if any, so that the situation of both parties can be taken into account.
Before the first appointment, you can think of questions in advance and write down information about your own and your partner's state of health, illnesses, medications, and previous pregnancies. This helps the physician to form an overall picture of your situation and plan targeted examinations.
An ultrasound scan is performed on the woman through the vagina. Depending on the situation, the examinations are supplemented with basic blood tests and hormone tests. Basic blood tests can examine, for example, haemoglobin, vitamin D, ferritin, and thyroid levels.
If necessary, hormone function can be investigated more extensively, in which case examinations may include, for example, AMH, or anti-Müllerian hormone, which provides indications of the egg reserve of the ovaries. The patency of the fallopian tubes can also be examined if necessary.
As a basic examination, a semen analysis is performed to evaluate the count, motility, and structure of sperm. If necessary, sperm DNA fragmentation or oxidative stress can also be investigated.
After the basic examinations, the physician will go through the results with you and answer your questions. The treatment options will be reviewed and an individual treatment path will be planned for you. In most cases, treatment can be started immediately. Sometimes, however, further examinations are needed.
If the initial examinations give cause, more detailed investigations can be carried out, such as chromosome tests, thrombophilia testing and screening for autoimmune diseases. Often, there is only cause for these examinations if infertility treatments have not been successful or miscarriages occur repeatedly.
The results of the examinations are reviewed together with a physician, and based on them, a treatment suitable for your situation is planned. Treatment options include, for example, ovulation induction, insemination and in vitro fertilisation. If necessary, donated gametes can also be used in the treatment.
