Infertility refers to a situation where pregnancy has not begun within a year despite a regular sex life. If the woman is over 35 years old, examinations can be started after just six months.
Approximately 15 per cent of couples suffer from involuntary infertility at some stage of their lives. Most often, it involves reduced fertility. Sometimes the reason remains unexplained despite thorough examinations.
Involuntary infertility is a deeply moving and challenging life situation for many couples. It can evoke strong emotions, such as sadness and the experience of loss.
Along the way, more difficult emotions can also surface, such as jealousy or bitterness. In such a situation, the importance of loved ones and peer support is paramount. They help in processing emotions and coping.
At Felicitas Mehiläinen, we thoroughly examine all factors affecting fertility. Our goal is to create the best conditions for the beginning of pregnancy and its safe progress.
Infertility can stem from various factors related to either the woman or the man. However, a woman's age is a decisive factor regarding fertility. Postponing childbearing to a later age is one reason for the increasing prevalence of infertility, as the likelihood of spontaneous conception declines sharply with age.
Per menstrual cycle, the probability of a 20-year-old woman conceiving is approximately 35%, compared to about 17% for a 30-year-old and around 6% for a 40-year-old. For a 45-year-old, the probability is only about 0.5%. The chances of success for infertility treatments also diminish with age.
The cause of infertility is still equally often female (25 %), male (25 %), found in both partners (25 %), or unknown (25 %). The most common causes of reduced fertility are:
In women, the most common cause of infertility is ovarian dysfunction. In this case, the maturation and release of the egg, or ovulation, is disrupted.
PMOS can cause ovulation disorders. An irregular or long menstrual cycle may indicate PMOS, or polyendocrine metabolic syndrome. PMOS may be associated with ovulation disorders that can make getting pregnant more difficult. The syndrome is also associated with hormonal and metabolic disorders.
Hormonal disorders can disrupt ovulation. Overproduction of the milk hormone prolactin and thyroid dysfunction can also cause ovulation disorders. Even mild thyroid dysfunction is treated when investigating infertility. Often, proper treatment of the thyroid disorder alone is sufficient for a pregnancy to begin.
Weight and exercise affect ovarian function. Ovarian function can also be disrupted by being overweight or underweight. A normal weight and regular exercise promote fertility. On the other hand, excessive exercise can make getting pregnant more difficult for some women.
Ovarian function can decline prematurely. Sometimes the cause of infertility can be a premature decline in ovarian function. In this case, the ovaries stop functioning before the age of 40.
Blockages and adhesions hinder the passage of the egg. Approximately 15 percent of women undergoing fertility examinations are diagnosed with fallopian tube damage. The fallopian tubes may be completely blocked, or their normal function may be disrupted, for example, due to adhesions formed by scar tissue. Adhesions can constrict the fallopian tube or alter its position, thereby hindering the passage of the egg.
Inflammations are the most common cause of blockages. The most common cause of a blockage is the after-effect of chlamydia or another infection. Operations in the pelvic area or endometriosis can also cause adhesions. If the fallopian tube is completely blocked, natural fertilisation is prevented. In this case, IVF treatment is used as the treatment.
In endometriosis, the mucous membrane of the uterus occurs as separate lesions outside the uterus. These lesions thicken and bleed in the same way as the mucous membrane inside the uterus.
Pain is a common symptom. Menstrual pain as well as pain during intercourse or vibration can be signs of endometriosis. Endometriosis is a common disease in women of fertile age and can reduce fertility.
Structural abnormalities of the uterus can complicate pregnancy. Structural defects of the uterus and muscular tumors growing towards the uterine cavity, i.e. myomas or polyps, can interfere with the onset of pregnancy and the course of pregnancy. Curettage of the uterine cavity and uterine infections can cause adhesions, which can make the onset of pregnancy difficult or even prevent it.
An abnormality in semen may be congenital, acquired later, or temporary. Therefore, an abnormal finding must always be checked before further examinations.
There can be many reasons for the deterioration of semen. The background of male infertility may include an undescended testicle, varicocele, injuries to the testicles, operations, or sperm antibodies. A previously contracted genital inflammation, such as chlamydia, can also be the cause. Certain medications have a deteriorating effect on semen quality. Most often, however, no clear cause is found for impaired semen.
The sperm count affects fertility. Male fertility begins to decline if there are fewer than 13 million sperm per millilitre of semen. There may not necessarily be any sperm in the semen, but they can still be found in a testicular sample.
Donor sperm can be used as a treatment. If the sperm count or quality is significantly reduced or no sperm are produced at all, the use of donor sperm can be considered in the treatment.
Diseases and lifestyles can increase oxidative stress. For example, smoking, high alcohol consumption, stress, and being overweight can increase the amount of oxygen radicals in semen.
Oxidative stress can impair sperm function. Oxidative stress can damage the surface structures, motility, and genetic material (i.e. DNA) of sperm. This impairs fertility. Improving one's lifestyle always has a positive effect. Even a minor improvement in the quantity and quality of sperm can improve the chance of fertilisation.
There may be abnormalities in the sperm's genetic material. In some cases, a sperm defect may be caused by damage to the sperm's DNA strands, i.e. DNA fragmentation. A sperm defect can also be caused by a deletion in the regions of the Y chromosome regulating sperm production, i.e. Y deletion, or another chromosomal abnormality. Examining the man's chromosomes is appropriate if there is very little or no sperm.
Fertility is affected by many factors, some of which you can influence yourself. Age is a significant factor, but lifestyle plays a major role in supporting fertility. Healthy choices can improve the chances of pregnancy. They also support the success of treatments.
A balanced diet, regular exercise and maintaining a normal weight are important. They affect both female and male fertility. Avoiding smoking and heavy alcohol consumption is recommended. Stress management and adequate sleep also have a positive effect. Good therapeutic balance of chronic diseases, such as diabetes or thyroid disorders, is of paramount importance.
If pregnancy has not begun despite regular attempts, it is advisable to seek fertility examinations. At Felicitas Mehiläinen, you can get an appointment with a specialist quickly and without a referral.
Consult a specialist if:
co-parenting
Co-parenting parenthood makes it possible to have a child in a situation where both parties wish for a child without a romantic relationship.
Fertility treatments for female couples and single women
Infertility treatments for female couples and single women are carried out with donated sperm, either through insemination or in vitro fertilisation t
In vitro fertilisation (IVF)
Are you considering IVF treatment? Felicitas Mehiläinen offers comprehensive IVF treatments, expert support, and a clear treatment path.
In infertility examinations, the health status of both partners and the factors affecting fertility are first examined. For the woman, assessments are performed of, for example, the menstrual cycle, ovulation, and the function of the uterus and ovaries. For the man, the quality of the semen is usually examined. Any necessary further examinations are planned based on the findings.
The cause of infertility may be related to the woman, the man, both, or remain unexplained despite examinations. Therefore, examining both helps clarify the situation as comprehensively as possible and find the most suitable treatment path.
Yes. You can seek examinations earlier if your menstrual cycle is very irregular or periods are absent. A known illness affecting fertility or a previous illness, surgery or inflammation can also be a reason to seek examinations earlier than usual. At over 35 years of age, it is advisable to seek examinations already before trying for a year.
Yes. Semen quality can vary due to factors such as illness, fever, medications, and lifestyle. Therefore, a single abnormal semen result does not necessarily mean a permanent decline in fertility, and the result can be checked if necessary.
Yes. Factors related to fertility can also be investigated before actively trying to get pregnant. Examinations can assess, for example, the menstrual cycle, ovarian function, and other factors affecting fertility. However, examinations cannot definitively predict a future natural pregnancy.
If examinations do not reveal a clear reason why pregnancy has not begun, it may be unexplained infertility. This does not mean that pregnancy cannot occur. Further treatment is assessed on the basis of the overall situation, and the plan is influenced by, for example, age, the duration of infertility, and previous examination results.
