See a doctor in public healthcare, such as a health centre, student health care or occupational health care. You can discuss your wishes and goals regarding your rehabilitation with the physician and how they could be integrated into your daily life. Based on the discussion, the physician will draw up a rehabilitation plan for your rehabilitation needs.
Before submitting the rehabilitation application, you must choose a therapy provider and agree on starting the therapy. Contact the person in charge of the location or call the rehabilitation advisory service at 010 414 0910 (local network rate/mobile charge). Together we will find a suitable therapy provider and place of implementation for you.
Once the start of the therapy has been agreed with the therapy provider, you can add the therapy provider’s information to the application. Fill in and download Kela’s KU 104 form for intensive medical rehabilitation here (pdf Kela.fi) External link.
Send the form and the rehabilitation plan to Kela either in OmaKela or by post. See more detailed instructions on how to submit the form and the rehabilitation plan to Kela (Kela.fi) External link.
You will receive the decision in writing by post to your home. Kela will also send the decision to us at Mehiläinen for information, in which case it also serves as a payment commitment and place reservation. We will contact you within 2 weeks regarding an initial appointment or a potential introductory visit.
