ICSI treatment is used at discretion in the following situations:
During ICSI treatment, the ovarian follicles are grown and matured, and the eggs are retrieved in the same way as in conventional IVF treatment.
The oocyte is fertilised by introducing a single sperm into the oocyte with a thin glass needle using micromanipulator equipment. With this operation, our experienced laboratory staff ensure that the sperm enters the oocyte, even if it is unable to move, attach to the surface of the oocyte, or penetrate the oocyte shell on its own. In this way, we facilitate fertilisation if the quality of the semen is not sufficient for conventional IVF treatment. The time of fertilisation is in the afternoon of the egg retrieval day.
In microinjection treatments, normally 70–100% of the oocytes are fertilised. To maintain good results, Felicitas Mehiläinen continuously monitors the laboratories' fertilisation results using international benchmarks as a point of comparison, ensuring the high quality of treatment. After fertilisation, the embryos are cultured in the laboratory, and the best ones are selected for embryo transfer and/or cryopreservation for later use.
Techniques suited for different situations have also been developed from the ICSI method, with which we strive to ensure the fertilisation of oocytes and embryo development in even the most challenging cases. We always choose the form of treatment carefully, best suited to your individual situation.
If no sperm are found in the man’s semen sample (azoospermia), but they are present in the testicular tissue, sperm can be collected directly from the epididymides or testicles (testicular biopsy). Testicular biopsy sperm can be used for fertilisation using the microinjection technique. The retrieval of sperm from the testicle can be done on the day of the egg retrieval, in which case the sperm are used for fertilisation fresh, or the tissue sample can be pre-frozen.
With the LAISS method, live sperm capable of fertilisation can be distinguished from immotile sperm. The LAISS method is useful in a situation where sperm are found in the semen, but they are immotile (asthenozoospermia or necrozoospermia), or if sperm retrieved from testicular tissue are used in the treatment. With the LAISS technique, a laser pulse is directed at the tail of the sperm, to which a living sperm reacts differently than inanimate sperm. The fertilisation capacity of sperm confirmed to be alive is significantly better than that of immotile, partially dead sperm.
The PICSI method aims to enhance sperm selection in microinjection treatment. PICSI is based on separating mature sperm from the semen. A mature sperm cell produces a specific molecule (hyaluronic acid, HA) on its surface, with which it normally binds to the cell zone surrounding the oocyte. With PICSI, the binding capacity of the sperm is tested, and mature sperm with the highest fertilisation capacity are selected for use in microfertilisation.
